West Coast Governors Form Alliance to Champion Science-Based Health Guidance
In a significant move signaling a rift with federal health policy, the governors of California, Oregon, and Washington have officially launched the West Coast Health Alliance. This new coalition aims to establish unified, evidence-based public health recommendations, particularly concerning immunizations, directly challenging what they describe as the politicization and erosion of scientific integrity at the U.S. Centers for Disease Control and Prevention (CDC) under the Trump administration. The alliance was announced on September 3, 2025, by the Democratic leaders of the three states.
Roots of the Alliance: Distrust in Federal Leadership
The formation of the West Coast Health Alliance stems from deep-seated concerns over recent federal actions impacting the CDC. Governors Gavin Newsom of California, Tina Kotek of Oregon, and Bob Ferguson of Washington issued a joint statement condemning what they characterized as the “Trump Administration’s destruction of the U.S. CDC’s credibility and scientific integrity.” They cited specific actions, including the “mass firing of CDC doctors and scientists” and a perceived shift from evidence-based science to “ideology.” This has led to a lack of trust in federal guidance, particularly regarding vaccine recommendations and accessibility. State health officials echoed these sentiments, with California Department of Public Health Director Erica Pan stating that “the dismantling of public health and dismissal of experienced and respected health leaders and advisors… are placing lives at risk.”
The alliance also follows controversial decisions by Health and Human Services Secretary Robert F. Kennedy Jr., including the removal of all 17 members of the CDC’s Advisory Committee on Immunization Practices (ACIP) and the subsequent appointment of individuals who have previously spread vaccine misinformation. Furthermore, recent federal policy changes had limited access to COVID-19 vaccines, restricting eligibility and removing recommendations for pregnant women, actions that the West Coast governors believe directly threaten public health.
Mission: Science, Trust, and Access
The core mission of the West Coast Health Alliance is to ensure that public health policies and recommendations within the participating states are guided by safety, efficacy, transparency, access, and trust, rather than political influence. The alliance intends to coordinate regional health guidelines, with an initial focus on developing aligned immunization recommendations. These recommendations will be informed by the expertise of trusted national medical professional organizations, rather than solely relying on the CDC’s evolving directives.
“Our communities deserve clear and transparent communication about vaccines — communication grounded in science, not ideology,” stated Sejal Hathi, Director of the Oregon Health Authority. The governors emphasized that the alliance aims to provide residents with consistent, science-based information they can rely on, regardless of “shifting federal actions.”
Expanding Reach: Hawaii Joins the Coalition
Following the initial announcement by California, Oregon, and Washington, Hawaii officially joined the West Coast Health Alliance, expanding the coalition to four states. Governor Josh Green of Hawaii expressed pride in standing with West Coast partners to ensure public health decisions are grounded in science. This expansion signifies a growing regional commitment to collaboratively address public health challenges and uphold scientific standards.
A New Era of State-Led Health Policy
The establishment of the West Coast Health Alliance represents a notable development in the landscape of public health policy, highlighting a proactive stance by states in response to federal leadership perceived as unreliable or politically compromised. It mirrors past instances where states formed regional alliances, such as during the COVID-19 pandemic for purchasing medical supplies and coordinating reopening efforts. By prioritizing science-driven decision-making and seeking guidance from established medical bodies, the alliance seeks to rebuild public confidence in health recommendations and safeguard the well-being of millions of residents across the Pacific region. The states have pledged to finalize shared principles in the coming weeks, reinforcing their commitment to a science-first approach to public health. This news marks a significant moment in the ongoing dialogue about the role of federal versus state authority in public health matters.
